New funding announced by the Welsh Government for the Intermediate Care Fund has been welcomed by the Mid and West Wales Health and Social Care Collaborative.
Earlier this week, the Welsh Government announced they would be investing a further £17.5 million in the Intermediate Care Fund this year to continue projects which have proven to be successful in helping older and vulnerable people remain in their own homes.
“This new funding from Welsh Government provides health and social care partners across the region with a great opportunity to build on real achievements over the past year in developing intermediate care services and keeping older people independent at home,” said Pam Marsden, Director of Social Care and Leisure, Pembrokeshire County Council.
“Our regional collaborative provides us with the means of working across statutory, third and independent sectors to improve the experience and quality of life of older people.”
Councillor Simon Hancock, Cabinet Spokesperson for Adult Services, Health and Well-Being and Equalities, Pembrokeshire County Council, added: “We have lots of evidence from last year on how focused investment in intermediate care services can have a beneficial effect on vulnerable older people and help them stay well within their homes following a crisis which required input from health or social care services.
“The new funds will provide an excellent opportunity for us to build on innovative approaches developed last year which put Mid and West Wales at the forefront of citizen-centred, preventative and rehabilitative care.”
The Mid and West Wales Health and Social Care Collaborative aims to support the further development of intermediate care services across Wales, reduce pressure on acute services, and address the issues of unscheduled admissions to hospital while supporting older people to live independently at home.
In 2014/15, the organisation delivered more than 70 programmes in partnership across Hywel Dda University HB and Powys teaching Health Board, the four local authorities , and the third sector.
Future plans include consolidating the notable local successes evidenced during 2014/15, and further refining the programme of work through implementing a regional approach to ensure consistency in levels and quality of service for people across Mid and West Wales.
